Bug in the ACCEPT 3.0 equation that causes the equation to predict the same exacerbation risk for individuals.

I have compiled an HTML file that describes this in more detail alongside the corrected equation. The updated intercept terms, slope, and intercept for moderate-severe and severe exacerbations are on the HTML file as well.
