In the state machine explanation for a node (http://design.ros2.org/articles/node_lifecycle.html), it says - "If an error that cannot be handled by the node/system occurs in this state, the transition to the fatal error state is taken."

There is no fatal_error state in the diagram, nor is it mentioned anywhere else in the document.

That's a reference to an old draft of the state machine. I've submitted a PR #155 to update the reference.
