[New Feature] Add ability to add the selected Proxy Server from a subscription to my servers list

Description

In the "Proxy Servers" page when the "Default Proxy Server:" is one of the subscription's proxy,

• Show a button next to it "Add to My Servers"
• Clicking that the normal add "Proxy Server Details" dialog should show up, with the fields pre-populated with the subscription's servers details
• For name take of the subscription + the number that we calculate on the normal Add Server, formatted "{SubscriptionName} {Number}"
