Import model (track CSV imports)

### Description
Add an Import model to track CSV imports at the household level.

### Acceptance Criteria
- Import belongs to a household
- Includes:
  - source (e.g. "csv")
  - original_filename
  - created_at
- Migration committed
- `db/schema.rb` updated
