No Terraform environment separation — single `environments/` directory

Description

infrastructure/terraform/environments/ exists but the separation between staging and production environments should be verified. Applying production Terraform from the wrong directory could overwrite production infrastructure.

Acceptance Criteria

• Separate staging and production environment directories with distinct state files
• CI/CD only applies production Terraform on explicit approval
• terraform workspace or directory-based separation documented
