fix(github app): Generate installation tokens each time

We were previously tracking installation tokens for the app inside the GitHubAppManager but this is resulting in weird expired token issues. It doesn't seem like we would be hitting the rate limit for installation tokens created (cap is in the thousands per hour), so to be safe let's just generate a new installation token each time it's requested.
