feat: add Bloch sphere, state city plot, and histogram visualizations#23

New viz/ package with pure SVG generation (zero external deps) for quantum simulation result visualization: measurement count/probability histograms, single-qubit Bloch sphere projection, and isometric 3D density matrix city plots with real/imaginary panels.
Handle fmt.Sscanf error return in darken() and fix gofmt alignment in bloch.go, histogram.go, and statecity_test.go.
