Improve release-facing documentation so future releases are easier to understand and maintain from the outside.
Scope
- document changelog discipline for future releases
- add or refine sample outputs that show what the tool produces
- tighten the known limitations section so parser and detector boundaries are clearer to readers
- keep language public-safe and externally credible
Acceptance Criteria
- docs explain how changelog entries should be maintained for future versions
- release-facing samples are present and easy to find
- known limitations are clearer and more useful to external readers
- documentation changes stay concise and avoid duplicating the README unnecessarily
