Claude Usage Dashboard opens to wrong page

Usage Dashboard (under the Claude tab) takes me here:

https://platform.claude.com/settings/billing

For me, that page shows my API/organization usage. The correct URL (for personal subscriptions) appears to be:

https://claude.ai/settings/usage

I found the root cause while testing against local Claude Code OAuth credentials. The affected credentials can contain subscriptionType: "pro" while rateLimitTier is still default_claude_ai, matching the upstream Claude Code credential shape documented in anthropics/claude-code#38909.

CodexBar was only using rateLimitTier to derive the Claude login method, so this credential shape failed to resolve to Claude Pro. That made the subscription check return false and caused the dashboard action to use the generic Claude dashboard URL.

The PR fixes this by parsing and preserving subscriptionType, preferring it for OAuth plan detection, and keeping rateLimitTier only as a fallback. This avoids a broad "all OAuth means subscription" shortcut and keeps non-subscription OAuth accounts from being misclassified. It also adds regression tests for the subscriptionType: "pro" + rateLimitTier: "default_claude_ai" case.

Fixed on main in 275c195. Root cause: Claude OAuth can report subscriptionType: "pro" while rateLimitTier stays generic (default_claude_ai), so CodexBar did not mark the account as a Claude subscription and opened the generic billing page.

The fix now parses/preserves subscriptionType, prefers it for OAuth plan detection, and has regressions for the OAuth mapper and the actual Claude dashboard menu URL (https://claude.ai/settings/usage).
