cannot exit plan mode

just noticed this on main (likely a regression from the recent refactor)

steps to reproduce:

1. start a new session and create a plan (:Opencode agent plan mode)
2. switch to build mode (:Opencode agent build) and send a message to execute the plan
3. agent stays in plan mode

I encounter this too, maybe due to breaking changes in Opencode 1.4.0.

I am also not able to switch model mid-session. After the first message is sent, it is impossible to change the agent or the model that was first used.
