Expand CI to run on ubuntu, macOS, and Windows via a matrix. Revamp CONTRIBUTING.md and README with platform-specific setup, installation options, a table of contents, usage examples, uninstall instructions, and improved developer workflow notes. Update src/mindmark/index.py to honor MINDMARK_HOME and use %LOCALAPPDATA% on Windows for the default index DB path (fallback to home on failure), improving cross-platform behavior and ergonomics.
