Created a new issue template to help organize issue-data when issues are created/submitted.

Looks good to me. We can always update this once folks start filing issues and we realized either a) one of the sections is not needed or b) additional section needs to be added.

In this project, we normally merge to dev, and then integrate that up to main. You can either do that here, or please integrate back to dev when this is merged.

Good point John about merging to dev first, I didn't catch that. Just for everyone's info, these templates are read from whatever the default branch is (in this case main), so we're all set. Confirmed that creating a new Issue brings up the template now.
