This PR updates the call ended reason documentation by correcting a typo ('occured' to 'occurred') in the provider fault error description and adds a new error code for Eleven Labs voice disabled scenarios. The new error code 'pipeline-error-eleven-labs-vapi-voice-disabled-by-owner' helps users understand when an Eleven Labs voice has been disabled by its owner. These changes improve the clarity and completeness of our call ended reason documentation.
