Knowledge-base primitives release. Three new tiers (fact, incident-narrative, incident-facts) + optional symptoms: frontmatter field on any-tier playbooks. Five new CLI commands. New audit --strict flag. LLM onboarding via /hit-em-with-the-docs:help and docs/LLM-GUIDE.md.

No breaking changes. A consumer with no knowledge-base subtree observes zero behavior change from 2.2.0.

Stats: 230/230 tests passing (was 134 at 2.2.0 baseline; +96 new). +6154 / -109 lines across 5 commits (PR #5).
