Pull request overview
This PR simplifies the pull request template to improve its usability and adoption. The updated template reduces complexity while maintaining essential review requirements.
Changes:
- Replaced verbose description section with direct "Describe your changes" prompt
- Consolidated two separate review checklists (SME and Documentation team) into a single, more actionable checklist
- Removed reference links to contribution guide from the template itself

Description
This PR template doesn't seem to be used so this PR creates a new PR template that is a bit more simple + straight forward, increasing actual use.
