[Tolk] Add a title to all Tolk code snippets outside of `/languages/tolk` that do not have one yet

This needs to be done for clarity and to safely enable Tolk highlighting with #1421

Two main kinds of titles are: "Tolk" and "<PATH_TO_FILE>.tolk"
