Better separation of review work and review milestones

Review state of the milestone is really separate thing. Each review has 2 days to be completed. For the sake of simplicity we combined the work and the review into a single milestone. Once we complete the work, the review time starts. Right now this is not clear from the button label.

For the sake of simplicity we combined the work and the review into a single milestone.

@vic-topcoder @acshields it didn't made things easy, rather it actually made things complicated in terms of implementation and understanding of the milestone. We started with complicated way and I think we ended up with simpler version where we separated out the work and review as two separate milestones. We already have this two separate milestone based timelines on production, please let us know if have any thing else to complete this task.

The current implementation with 2 separate milestones (design work - copilot; design review - customer) works better for our customers. I'm not sure what this issue addresses? Current implementation is good.
