Fixed

• Codex multi-agent tools now honor Task permissions. The
  skill-permission guard canonicalizes spawn_agent, wait_agent,
  send_input, close_agent, resume_agent, and their multi_agent_v1
  runtime names to the existing Task permission, so required reviewer
  subagents are not blocked by a valid high-risk task contract.
