Rewrite all documentation (moduledocs, function docs, README) to professional declarative tone
Enhance CI matrix with dedicated format, credo, test, and dialyzer jobs
Test across Elixir 1.15, 1.17, 1.19 with OTP 26, 27, 28

Test plan

mix test passes (2 doctests, 24 tests, 0 failures)
mix docs renders correctly
CI passes on all matrix combinations
