Patch release for the rerank defaults and MCP retrieval path.

  • Added configurable rerank_mode with default relevant_only, restoring the pre-0.4.3 behavior.
  • Fixed vector floor being dropped in the MCP search/answer path after rerank.
  • Kept search and answer rerank behavior symmetric.
