health-report: fold data-model/impl notes into new doc, drop stale root page Add verified Persistence/data model and Implementation notes sections to new/health-report.md (report_result schema, entry-point classes, email template, ITs), sourced from dataquest-dev dtq-dev. Remove the outdated root health-report.md, which documented the removed legacy healthcheck script and date-based report-diff.
