Custom fieldtype path incorrect in custom-markup.md

Instructions for creating custom fieldtypes in a custom form are missing the Fieldtypes directory in the path.

Hi @snerpton I have just reviewed the PR and can confirm these changes. 💪
I have tested this for latest v13 version and the fieldtypes views need to be in the fieldtypes folder for them to work.

I have added a suggestion to make this more obvious that it need to be in fieldtypes folder, can you let me know what you think about it and if you are up for making the same change on v10 and v13 of forms(as the default themes have the same folder structures meaning they follow the same pattern)? 😊
