Broken icon displayed on migration templating page during database errors

Describe the bug
When a database error occurs on the migration templating page, the icon displayed alongside the error message is broken or not rendered correctly.

To Reproduce
Steps to reproduce the behavior:

1. Open the migration templating page in a Wheels application
2. Trigger a database error
3. Observe the UI after the error occurs
4. Note that the icon shown with the error message is broken or missing

Expected behavior
When a database error occurs:

• A proper error icon should be displayed
• The icon should render correctly and match other error states in the application
