Outline panel no longer shows nested RSpec contexts/specs in v0.5.6

After updating the Zed Ruby extension from v0.4.6 → v0.5.6, I noticed that my RSpec outline is now flat: only the outermost describe names appear, and nested context and it nodes no longer show up.

This, of course, impacts navigation in large spec suites.
