scaffold v3.30.0 — guides coverage expansion

Expands scaffold guides from a single proving-ground guide to 11 guides covering every major subsystem, with verifiable file:line citation provenance gated by make check-all. All changes are additive.

• Eight new authored guides + two migrated. scaffold guides now ships pipeline, concepts, cli, install, review-workflow, multi-agent, dashboard, and knowledge, plus migrated Build Observability and Knowledge Freshness references — joining mmr for 11 total.
• :cite citation directive — :cite[path:line] (blocking) / :cite[…]{mode=advisory} (warn-only). The citation-drift gate discovers all guides dynamically.
• content/guides/AUTHORING.md — the guides markdown-dialect spec.
• Inter-guide relative-link validation in the guide build.

Changed

• build-freshness-reference.mjs retooled to deterministically regenerate the knowledge-freshness guide's live-data blocks — fixing the long-standing rebake citation-drift that kept make check-all red.

Migrated / removed

• The three legacy docs/**/reference.html pages retired to redirect shims.

Every guide was multi-model (MMR, 4-channel) reviewed during authoring.
