Add hook builder submission process

What changed

• require complete launch model submissions through pull requests
• document selection, licensing, attribution and version policy
• publish the fixed 0.80% creator, 0.10% builder and 0.10% Programmable allocation
• add a model acceptance record
• remove the idea-only launch model issue path

Why

External builders need one public submission path with clear requirements and no implied promise of acceptance or
revenue.
