RC.4 external-model study protocol freeze v3: crash-safe execution

V2 is retired before source access, router reveal, or model execution after an independent crash-recovery audit. V3 adds durable reserved / invocation-committed / response-persisted / completed states, terminal crash synthesis after a call may have started, and the V13/V14 crash-safe one-reveal protocols. It forbids duplicate paid calls after an uncertain crash.
