修复：externalUsageLog 为空（默认值）时误跳过读取，导致 vision-fallback 写入的 usage.jsonl 从不被统计。现在只有显式 "off" 才禁用。\n\n安装：dsh plugin --profile web add dsh-usage-dashboard-plus@0.2.2
