ResolvedTranscript & RequestBuilder (Extra)

• Transcript.resolved() — normalises a raw Transcript in a single pass, pairing toolCalls entries with their toolOutput entries into ToolInteraction pairs. Tracks the latest GenerationOptions and ResponseFormat for easy access when building provider requests.
• RequestBuilder protocol — foundation for provider-specific request builders. Includes a segmentsToText(_:) default implementation that converts .text, .structure, and .image segments to a plain string.

GenerationSchema improvements

• Added typeName property: carries the Swift type name through GenerationSchema, enabling Transcript.ResponseFormat.name to reflect the actual type instead of the static string "schema-based".

Transcript description improvements

• Transcript.ResponseFormat.description now emits the full JSON Schema for easier debugging.
• Improved CustomStringConvertible output for Instructions, Prompt, ToolCalls, ToolCall, ToolOutput, and Response.

Removed

• Transcript+Extraction.swift superseded by Transcript+Conversion.swift.
