EXP workflow fixes

  • Open tasks now receive planned EXP before completion; completing a task promotes that value to earned EXP.
  • Natural-language @exp requests no longer confuse missing planned EXP with incomplete work.
  • Startup reconciles open tasks that missed automatic scoring, while @exp unscored and @exp sync remain available.
  • Automatic scoring failures persist instead of disappearing.
  • Completion tracking ignores excluded internal paths before observing them.

Plan mode

  • Adds Ctrl+Tab plan-mode switching, intent detection, and the /plan command.
