OAuth lifecycle improvements

Reactive xAI OAuth refresh after authentication failures, including rotated refresh-token persistence.
Antigravity reuses OpenCode Google credentials and local agy-auth.js OAuth client configuration without OpenCode running.
Both integrations retry quota collection after token refresh and preserve last-good quota data on failure.
Native failure notifications and regression coverage for both providers.
