[Addressbook]Update description for import csv dialog with chainID colummn

Title/Description

The csv file contains 3 columns but the info on the import window describes only 2

Steps to reproduce

1. Go to 'Addressbooks
2. Click on Import
   Current result: the csv description contains only "Address" and "Name" , but the actual file ( exported csv) contains also chainid

Expected result

Add info about chainid to the csv description
