Windows: "cannot execute binary file" on hook invocation (#35)

Root cause: Claude Code already spawns a shell to execute hook commands. The redundant sh prefix in hooks.json was misinterpreted as a script filename (not a command), causing sh to try executing /usr/bin/sh as a text script — resulting in "cannot execute binary file".

Fix: Removed sh prefix from all hook commands. The #!/bin/sh shebang in hook-wrapper.sh handles interpreter selection correctly on all platforms.
