Fix: On macOS 16 (Tahoe), fall back to /bin/cat and /bin/mv (Apple platform binaries, TCC-exempt) when POSIX open() is blocked by TCC. If all strategies fail, shows a clear error message with instructions to grant Full Disk Access to Terminal.app.
