🔧 Compatibility

Improved OpenTelemetry token usage reporting to match updated semantic conventions.
Added cache token usage tracking for Anthropic Messages streaming.
Added cache_creation.input_tokens telemetry for chat completions.
Fixed AI SDK AbortError and TimeoutError handling so they return 504 responses.
Improved client disconnect telemetry by recording the abort reason.
