Fixed

  • 12-QA phases skipped — Phase 3 and Phase 4 auto-verify exits used ambiguous auto-proceed which LLMs interpreted as "jump to next numbered phase", skipping Spec 12-QA (Phase 3.5) and Plan 12-QA (Phase 4.5). Replaced with explicit phase references and "do NOT skip" instruction.
