Added

• Added an optional plan argument on ExitPlanMode (desktop, CLI, and Native Worker) so the agent can hand off complete plan markdown when the plan file has not been written yet, including UI-toggled plan mode that never ran EnterPlanMode.
• Added a Request revision dialog on the in-chat plan review card so users can send feedback, mark the plan rejected, and send the agent back into plan mode without a separate side panel.

Changed

• Removed the standalone Plan panel and folded review, implement, new-session execute, and revision into the chat review card.
• Native Worker now provisions or repairs a session plan file on enter/exit instead of rejecting legacy rows without a path, and plan-mode turn context tells the model to finish through ExitPlanMode rather than printing the plan as ordinary reply text.
• Windows ARM64 Native AOT publish prefers lld-link (and CI moved the ARM job to windows-11-vs2026-arm) so MSVC 14.44 no longer fails the large worker object with LNK1322.

Fixed

• Fixed plan review cards that went blank after a dormant-memory content drop or a missed plan/ui-update by reading the plan file (including SSH) and reloading the SQLite row when the session returns to idle.
• Fixed Request revision doing nothing when the in-memory plan entry was missing by reloading the session plan from SQLite first.
