• install-docker.ps1 removes stale apx-v1 container before docker compose up -d (parity with .sh)

• Audit integrity diagnostics: distinguish corrupt log lines vs hash-chain break in apx_doctor, apx_ctl integrity, and /health
• Upgrade runbook: recovering degraded health; API key hints for in-place upgrades from v1.2.0
