• createAcpRuntime({ agent }) now works without a host. The runtime defaults to approving tool permissions once and selecting the first offered auth method; pass an explicit host for production policy, UI prompts, file system/terminal capabilities, logging, or wire middleware.
• PermissionDecision constants (AllowOnce, AllowAlways, Deny) for host permission decisions, while keeping the existing string literals backward compatible.

• Corrected permission-decision docs to use the actual supported deny value ('deny') instead of non-existent deny_once / deny_always variants.
• Improved npm package metadata and README wording so searches for Agent Client Protocol framework/runtime terms can discover the package more reliably.
