@acp-kit/author-reviewer-loop TUI no longer crashes with RangeError: Invalid array length when a pane's flow ends on a tool item. isBlankTextFlow(undefined) previously returned true, causing the tool-run coalescing loop in visibleFlowRows to never terminate at end-of-flow and grow skipped until allocation failed. The helper now returns false for missing items so the loop stops at the array boundary.
