fix(chat): prevent first message from staying in sending state - #664

Assign a stable message ID and seed optimistic content for every lazy-created chat session. Delay session activation until prompt acceptance so failed first sends retain the draft without leaving a ghost message.
Merge the upstream workflow, provider, ingest, and model pricing updates while preserving the stable first-message ID fix in SessionPage.
