Hardened Matt grill/TDD release.

Added

  • Mandatory pre-design grill.md gate using the full Matt grill-with-docs workflow.
  • Mandatory post-design design-review.md gate using the full Matt grill-with-docs workflow.
  • Project-local Matt tdd skill with vertical RED/GREEN/REFACTOR apply discipline.
  • Root CONTEXT.md glossary support plus optional CONTEXT-MAP.md guidance.
  • ADR 0005 for the hardened Matt grill/TDD lifecycle.

Changed

  • Lifecycle is now proposal -> specs -> grill -> design -> design-review -> adr -> test-plan -> tasks -> apply -> verify -> archive.
  • test-plan.md now requires vertical TDD slices and RED/GREEN evidence instead of soft verification placeholders.
  • /opsx:continue, /opsx:ff, /opsx:apply, /opsx:verify, installer, docs, schema, and overlay checker are updated for the hardened gates.
