Ollama model management via slavemode

Adds three new slavemode capabilities for remote Ollama management:
- slavemode.ollama-list  — list installed models with name/size metadata
- slavemode.ollama-delete — delete a model by name
- slavemode.ollama-pull   — pull a model with streaming progress reports

Auto-enables these three caps on first launch when Ollama is detected
(slavemode-allowed-caps absent from config + any llm.* cap present).

Replaces hardcoded OLLAMA_DELETE_URL/OLLAMA_PULL_URL constants with
get_ollama_base_url(), which reads ollamaBaseUrl from agent config and
falls back to http://127.0.0.1:11434. All Ollama functions (including
exec/llm.py, exec/custom.py, capabilities.py) now build URLs at call
time so a remote/custom Ollama host is respected everywhere.

Management frontend: new OllamaManagerModal with model list, per-model
delete, and pull-by-name with 30-minute timeout on the agent card.
