[UAT] #53 second disposable Root-to-Codex trace capture fixture

[Alive24]

Disposable operator-approved fixture for FailureReport #53 Halo Research.

This is a second fresh fixture because the prior disposable fixture correctly created persistent FailureReport session state in its target checkout before a transient local API failure interrupted finalization.

Diagnostic request:

• Inspect the committed sparse-evidence fixture under eve/evals/ckb/fixtures/issue-45/ in the exact target checkout revision supplied to the run.
• Determine what the retained Issue, linked fix commit, and patch establish.
• Separate verified facts from plausible interpretation and unknowns.
• Recommend the smallest next evidence-gathering action.

Guardrails:

• Do not claim a reproduced root cause, a specific unretained runtime error, or verified end-user success.
• Do not change business code or route project work.
• The only expected GitHub mutation during the capture is FailureReport-managed append-only workpad activity on this disposable Issue.

This Issue may be closed after #53 Halo Research is complete.
