[UAT] #53 phase-aware exploration Root-to-Codex fixture

[Alive24]

Disposable operator-approved phase-specific fixture for FailureReport #53 Halo Research.

This fixture is reserved for one configured Root-to-Codex capture phase so exploration and exact-candidate validation do not reuse target-local FailureReport runtime state.

Diagnostic request:

• Inspect the committed sparse-evidence fixture under eve/evals/ckb/fixtures/issue-45/ in the exact target checkout revision supplied to the run.
• Determine what the retained Issue, linked fix commit, and patch establish.
• Separate verified facts from plausible interpretation and unknowns.
• Recommend the smallest next evidence-gathering action.

Guardrails:
