• packages/mcp-adapter/src/index.ts slimmed from 496 → 40 lines. The tool catalogue (every tool's schema, description, the ITEM_SCHEMA constant, the AUTHORING_HINT and ASK_USER_CROSSREF cross-tool prose) moved verbatim to a new tool-definitions.ts. No behavior change — same tools array exported, same shape. Just a much clearer landing pad for tool authoring/copyediting work, which is the most-frequent edit target in the adapter.
• session-id.ts resolver gets test coverage. Six hermetic tests in packages/mcp-adapter/test/session-id.test.ts lock down the fallback chain that produced v0.3.52's silent regression: env CLAUDE_CODE_SESSION_ID override, legacy CLAUDE_SESSION_ID fallback, hook-file at matching ppid, ppid-mismatch falling through to transcript scan, transcript scan picking the newest .jsonl, and the all-empty undefined case. Each test isolates its own tmp HOME — no host-env pollution.
• Small refactor to enable hermetic testing: resolveClientSessionId now accepts optional { homeDir, cwd, ppid, env }. Production callers pass no args; defaults wire to homedir() / process.cwd() / process.ppid / process.env unchanged.
