Release 0.1.4 — three-level canvas hierarchy

Replace Paper/Mindmap.canvas with a navigable hierarchy:
  Paper/Projects.canvas → Paper/Projects/<name>.canvas → Paper/Papers/<name>.canvas

Each level uses a radial, graph-view-style layout. Glossary terms
shared by multiple papers appear once with edges to each paper.
Stale canvas files (including the legacy Mindmap.canvas) are pruned
on re-sync. Layout is deterministic: same sync data → identical JSON.
