Support Plex movie versions, editions, and combo-pack matching

Add Plex-aligned output naming for movie versions such as 4K and 1080p, and editions such as 3D. Keep 2D movie files in the base movie folder while placing 3D rips in a separate Plex edition folder. Match combo-pack layouts where separate 4K, Blu-ray, and 3D film discs should organize into the correct base and edition folders. Suppress duplicate missing bonus-feature entries when the same extra is matched from another disc. Document the v0.9.0 release notes in the changelog.
