v3.0: Renames LogEntry to ChangeLog

Renames the `LogEntry` entity to `ChangeLog` for clarity
and better reflects its purpose as a record of changes.

Updates the entity's properties to store information about
the identity that made the change, as well as adds indexes
to improve query performance.
