Summary

The previous five operator-mind phases successfully detect and report failure modes. A real session this morning showed they're not enough on their own:

Phase 2 refused a `PORT=25632 npm run dev` spawn because inotify was at 124/128. The model read the refusal, decided the cleanup was a USER action, and politely told the user "you can run pkill ... to clean up" instead of running it itself. The Artemis task ended undone for a problem KCode could have fixed in two seconds.

The gap is not the detection — that part works perfectly — it's the language of the refusal. Phrasing the recovery as "Options:" makes the model treat it as suggestions for the user. Phase 6 fixes that.

What changed

1. AUTHORIZED RECOVERY blocks in phase-2 refusals

Both refusals (port collision + inotify saturation) now end with an explicit AUTHORIZED RECOVERY block:

The block:

  1. Names specific commands the model is pre-approved to run as its next tool calls.
  2. States in the block itself that the user does not need to be consulted.
  3. Lists the steps in order with a final "retry the original command" step so recovery without retry is structurally impossible to forget.
  4. Provides an ALTERNATIVE clause for cases where recovery might harm something the user wants alive.
  5. Explains why the named commands are safe ("touch only ephemeral dev-mode watchers, never user code/files/git").

2. New system-prompt section: buildOperatorRecoveryGuidance

A new high-priority block in the system prompt explicitly teaches the model how to read AUTHORIZED RECOVERY:

  • Execute the steps yourself, in order. Do not paste them into a message for the user. Do not ask "should I run this?"
  • Do not invent your own variations. If the block says `pkill -9 -u $USER -f 'next-server|bun --watch|nodemon|vite'`, run exactly that.
  • Run the retry step (the original failing command) after recovery. Recovery without retry is incomplete.
  • If the block has an ALTERNATIVE clause, prefer the AUTHORIZED RECOVERY path first.
  • If the recovery itself fails, THAT is the moment to ask the user — not before.

Ends with: "You are the operator. Operate."

Test plan

  • `bun test src/core/operator-recovery.test.ts` — 6 / 0
  • `bun test src/core/bash-spawn-preflight.test.ts` — 9 / 0 (updated for new language)
  • `bun test` (full) — 6106 pass / 11 skip / 2 fail (2 fails are pre-existing `file-sync` `EMFILE` flakies, unrelated)
  • `bun run build` — production binary deployed to all 3 paths
  • End-to-end port collision: real `Bun.serve()` on ephemeral port → preflight refuses → report contains AUTHORIZED RECOVERY block with exact `kill PID` command and ALTERNATIVE clause
  • Manual smoke: trigger the original Artemis scenario (saturated inotify + dev-server spawn) and verify the model now runs `pkill` and retries instead of asking the user

What this does NOT change

Phases 1-5 are unchanged. Their detection logic is intact. Phase 6 only changes how refusals talk to the model and adds a system-prompt section explaining the convention. No new probe, no new history, no new tool wiring.

     might harm something the user wants alive.
  5. Explains why the named commands are safe ("touch only ephemeral

Refusal language updated for both phase-2 checks:

  * Port collision: kill <occupant PID> → sleep 1 → retry. Alternative:
    pick a different port.

  * inotify saturation: pkill -9 -u $USER -f 'next-server|bun --watch
    |nodemon|vite' || true → sleep 1 → retry. If saturation persists
    the leaks are owned by another UID and the user is asked to run
    sudo sysctl -w fs.inotify.max_user_instances=1024.

System prompt layer added (src/core/system-prompt-layers.ts):
buildOperatorRecoveryGuidance(). High-priority section that explicitly
teaches the model how to read AUTHORIZED RECOVERY blocks: execute the
steps yourself, do not paste them into a message, do not invent
variations, run the retry step, only escalate to the user if the
recovery itself fails. Ends with the directive "You are the operator.
Operate."

Tests:
  * 6 new cases for buildOperatorRecoveryGuidance covering each rule
    in the guidance.
  * Updated bash-spawn-preflight.test.ts to assert the new
    AUTHORIZED RECOVERY structure (Step 1/2/3, ALTERNATIVE, etc.)
    instead of the old "Options:" wording.
  * inotify refusal shape test that runs only when the host is
    actually saturated (no-op on healthy CI).

Suite: 6106 pass / 11 skip / 0 new failures.

Bumps version to 2.10.41.

Three MEDIUM-severity fixes from the security audit:

### Fix #6: Harden hook command validation (hook-executor.ts)
Previous validator had bypassable regexes:
- "curl ... | sh" blocked, but "curl ... | /bin/bash" passed
- "curl ... | $SHELL" passed
- Base64 decode to any interpreter passed

New patterns also block:
- Pipe to ANY shell/interpreter (bash, zsh, ksh, dash, fish, eval,
  source, exec, python, perl, ruby, node, $SHELL, /bin/*sh)
- authorized_keys writes (SSH persistence)
- crontab -e/-l (scheduled task persistence)
- Writes to .bashrc/.zshrc/.profile (shell init persistence)
- curl/wget to non-standard ports (C2 channels)

### Fix #7: Whitelist env vars for MCP servers (mcp-client.ts)
Previously MCP subprocesses inherited ALL process.env including:

Now only whitelisted env vars (PATH, HOME, USER, LANG, TZ, TERM, etc.)
are passed. Server-specific secrets go through config.env explicitly.

### Fix #8: Document silent catch blocks (3 files)
4 catch {} blocks in production code made failures invisible. Fixed:
- auto-test/detector.ts: now logs at debug level when package.json
  parse fails
- web-fetch.ts (2 catches): added comments explaining intentional
  silence in IP parser fallthrough
- enterprise/oauth/flow.ts: added comment explaining expected port
  scan failures

- Web engine writes .kcode-generated marker with siteType/stack/timestamp
- `kcode audit` detects marker and auto-enables --skip-verify (fixes 3h+
  scans on fresh scaffolds against local models)
- Scanner ignores pattern matches inside JSX className="..." and Tailwind-
  style string attributes in .tsx/.jsx files
